For hypertension and type 2 diabetes

Keep the week from getting lost.

HyperCare turns the readings, medication notes and symptoms you record into one clear Health Letter for the next doctor conversation.

The missing record

The facts exist. The record does not.

A blood-pressure reading stays in a notebook. A glucose result stays on a meter. Medication information is remembered or sent in a message. A symptom is noted without a date—or not at all. Before the next appointment, someone has to reconstruct the week.

HyperCare keeps the recorded week together.

01

Record the week.

Enter readings from a cuff or glucose meter, medication status, symptoms and context.

02

Review the Health Letter.

See what was recorded and what information is missing in one factual page.

03

Bring the week to the conversation.

Share a PDF with a doctor or an image with family when you choose.

Not another dashboard

One page that holds the week together.

A factual, patient-generated weekly health summary for the next doctor conversation. It does not diagnose, grade results or recommend treatment.

01

Shows how much information was recorded.

Coverage stays visible, including missing entries.

02

Brings the week’s context together.

Readings, medication, symptoms and optional activity context stay in one factual view.

03

Preserves what to discuss next.

The person chooses whether and when to share the Letter.

Every Letter keeps the app’s safety footer: HyperCare does not diagnose, monitor emergencies, change medication or replace a doctor.

Our direction

A fuller, person-owned health history between appointments.

Home readings tell only part of the story. HyperCare brings together measurements, medication, symptoms and optional activity context—so families can preserve what actually happened, not just isolated numbers.

Questions

What to know before using HyperCare.

What do I need to use HyperCare?

An Android phone and the cuff or glucose meter you already use. Readings are entered manually; no wearable is required. Optional Health Connect activity data can add context to the week.

Does HyperCare provide medical advice?

No. HyperCare organizes information that a person or family records. It does not diagnose, recommend treatment, adjust medication or monitor emergencies.

Where is information stored?

Health information stays locally on the phone. There is no HyperCare account or health-data cloud. Information leaves the phone only when the person chooses to share an exported Letter.

How do I get Android access?

Use “Get Android access” and leave your email. Priyanshu will reply with installation and setup details for the first recorded week. Do not include medical information in the form.
